I don't think the pink blob guy from DBZ really had any frame of reference. Any character who would kill or manipulate their family for their own goals or ideals is very ruthless. I also think Kurama from YuYu Hakusho is a ruthless character. He knowingly killed a child to get to his team's goal, so the rest of the team wouldn't have to and then he put an enemy that couldn't die into a permanent nightmare plant- not to mention using plants to kill his enemies from the inside out. The scene with her siting there slowly smoking a cigar and watching while that kid bleeds to death is seriously ruthless on a very personal level. While Kid Buu might destroy solar systems, he doesn't look his helpless victims in the eyes while they die. Besides if you want to talk mass slaughter, in Bokurano the group hiding behind the scenes kills off entire universes based on seriously messed up gladiator style contests. Griffith (Femto) from Berserk4). Mariko from Elfen LiedI'd go with Johan from Monster, but I haven't seen too many anime. The fact that he's placed second to someone intrigues (and scares) me.
