In other words technically not alive in the real world. Soul reaper can only interact with people in the real world via gigai (fake bodies) created by uhahara. Unless he;s that good and made the fake bodies fully functional. soul reaper should not be able to have a kid in the real world. you don't even hear that happening in the soul society. Maybe she was like Orihime and could see/interact with soul reapers? Maybe a quincy? haha .. going against her people's wishes when she was like virtually the last one besides Uryuu. hmmmmm.. A Soul Reaper is explicitly a usually human ghost with strong spiritual powers. The Soul Society is literally the afterlife, and when a Whole dies without being killed by a Quincy or a Hollow, barring special circumstances such as death by Hogyoku, their bodies break down into Reishi and the essence of the soul reincarnates into a new, living person. That does raise the question of what happens when Orihime rejects a Soul Reaper's death, though. It's something interesting, but I don't want to think too hard about it now, I'm still hurting from working out EXACTLY how Tsukishima's Book of the End works in all of the functions that it's demonstrated up to now. But they could not be together since he had a spiritual body which she could not sense. And they could not have a true relationship if he had to use a Gigai.Ishin, however, was bright, powerful and talented enough to learn how to use a Forbidden Kido Spell to enable him to return to a true human body.Using this forbidden kido spell caused him to lose his soul reaper powers. It also caused him to be banished from the Soul Society.Recently his powers were restored - perhaps with the help of Urahara.Note that Forbidden Kido Spells are referred to in the series.
